First Visit

We are always excited about meeting new patients during their first visit to our office! This important 60-minute visit will allow us time to complete a thorough exam, discuss your treatment options, including the estimated treatment time and cost of treatment. You will hear about exciting new technologies which are making treatment quicker and more comfortable than ever before! We will also discuss financing options designed to work within your budget. This first visit is complimentary – it is Dr. Travers’ gift to you!

In addition, if Dr. Travers determines that this is the appropriate time for you to start orthodontic treatment, we have reserved time following your exam to complete diagnostic records. These records include X-rays, photos and impressions for study models. This is necessary to develop a treatment plan customized just for you.

Please assist us by providing the following information at the time of your first visit:

• Please download the forms listed below, print and complete them, then bring them with you or fax them to our office at (972) 910-8203.
• If you have orthodontic insurance, bring your insurance card.
• Any panoramic X-ray taken within the past six months.
